How do I keep my coverage after I leave employment? 

Answer:  If you have had at least one premium payroll deducted prior to leaving employment, your coverage will automatically be changed to direct bill.  You will receive a bill from AFR Life.  If you choose to keep your coverage, you may pay AFR Life directly.  If you do not wish to keep your coverage you may let the coverage lapse or you may call AFR Life at 800-425-9303 to request cancellation.

How do I set up my policy on Automatic Bank Draft?

Answer: Access the “AFRL Bank Draft Form”.  Please complete this form and mail to PO Box 21061, Oklahoma City, OK 73156 or fax to 405-218-5586.

How do I file a claim?

Answer: Please call us at 800-425-9303 to speak with our Claims Advocate.  The Claims Advocate will walk you through what is needed and answer any questions you may have.  You can find the AFR Life Claim Form in the Forms section of this website.  You may mail the completed form to PO Box 25968, Oklahoma City, OK 73125 or fax to 405-218-5586.

How do I make changes to my policy?

Answer: If you have questions regarding what options are available, please call us at 800-425-9303.  You can access the policy change form in the Forms section of this website.  

How do I change my beneficiary information?

Answer: Please complete AFR Worksite Change Form located in the forms section of this website.  You may mail the completed form to PO Box 25968, Oklahoma City, OK 73125 or fax to 405-218-5586.
